| Create a Birth Plan- Be Prepared! Here is a simple form to use, or write your own. Remember that if you do NOT want a procedure to specify that you MUST be asked for permission if any deviation from your Birth Plan occurs. In some states failure to follow written/signed instructions constitutes battery. Also make sure you and your Dr have signed (and your partner, if giving permission to make decisions for you) and make copies of the SIGNED form... at least 7- give to Dr for file, to family, to hospital the week BEFORE birth and keep copies in various places at home. Remember to note that at no time will the baby be taken alone anywhere, if breastfeeding that NO pacifiers or bottles be given, and if they cut the baby's cord too quickly, ask them to bank the baby's cord-blood. |
